On or about 10/12/12 at Sandersville, GA:  Employees were exposed to fall hazards while working at a construction site. The employer failed to ensure that mid rails of guardrail system were maintained in such a manner as to protect employees from fall hazards:  a. West End:  The mid rail of a wire rope guarrdrail system was measured at 4 inches above the ground.  The employer failed to maintain the wire rope guard rail system so that the mid rail was measured at a height of 21 inches or half the distance to the top rail.  b. East End: The mid rail of a wire rope guard rail system was measured at 4 inches above the ground.  The employer failed to maintain a wire rope guard rail system so that the mid rail was measured at a height of 21 inches or half the distance to the top rail.  c. Window Frames:  Window frames (3) were installed without a standard midrail.  The employer failed to ensure that the window framer install adequate midrails prior to the date when the glass is installed into the frames.
