29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. WORKSITE: The employee on the left end of the roof bracket scaffold, repairing the skylight was not provided with an adequate fall protection system, exposing him to a 17-6 fall to the ground.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. WORKSITE: The employees installing shingles on the rake edge of a 6 in 12 pitch roof were not wearing fall protection, exposing them to a 19 foot fall to the ground.

29 CFR 1926.502(d)(15)(i): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two. WORKSITE: The anchorage used by employees as part of a fall arrest system was not properly installed, exposing the employees to a 176 fall to the ground.
