29 CFR 1910.106(d)(7)(iii): Open flames were permitted in areas where flammable liquids are being stored; a) On or about 10/15/12 in the storage room of the salon; a butane fueled hot plate was approximately 3-4 feet from the storage of four gallons of acetone and adjacent to a sink where acetone was being poured to fill smaller containers resulting in a fire and employee injury. 29 CFR 1910.106(d)(7)(i)(a): At least one portable fire extinguisher having a rating of not less than 12-B units was not located outside of, but not more than 10 feet from, the door opening into any room used for storage. a) On or about 10/15/12 at storage area at Manoa Nails salon; 3-4 gallons of acetone were in the storage area of the salon and the closest portable fire extinguisher was located outside the shop on the north wall near the 2nd floor exit.
