29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Jobsite, 1516 Superior Avenue, Pittsburgh, PA: Two employees were working on a residential roof and using a 16 foot Werner extension ladder to access the roof and the ladder only extended about 10 inches above the upper landing surface. The fall hazard to the employees was approximately 22 feet to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Jobsite, 1516 Superior Avenue, Pittsburgh, PA: On or about October 15, 2012, two employees were working from a residential roof without fall protection and were exposed to a fall of approximately 25 feet to the ground below. Affordable Roofing and Gutters was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 505178, citation number 2, item 1 and was affirmed as a final order on September 18, 2012, with respect to a workplace located at 7330 Bennett Street, Pittsburgh, PA 15208. Affordable Roofing and Gutters was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 310290127, citation number 1, item 1 and was affirmed as a final order on December 4, 2007, with respect to a workplace located at 4127 Perrysville Avenue, Pittsburgh, PA 15212 .
