Dayton Way, Building #371-378, Denver, CO 80231: On 10/2/12, the controlling and exposing employer did not ensure that employees eyes were protected while working with a pneumatic nail gun. Employee was not wearing safety glasses. Condition exposed employees to the hazard of projectiles to the eyes. Dayton Way, Building #371-378, Denver, CO 80231: On 10/2/12, the controlling and exposing employer did not ensure that fall protection systems were utilized by employees engaged in re-roofing activities while working at the unprotected edges of a roof on a single family residence. Employees were exposed to a fall hazard of approximately 13 feet. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.1053(b)(1): Because of the portable ladders length, the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in the mounting and dismounting the ladder: (a) Mateo Primo at 2685 S. Dayton Way, Building #371-378, Denver, CO 80231: On 10/2/12, the controlling and exposing employer did not ensure that employee was utilizing portable ladders in such a way that enabled them to access and egress from the roof using ladders that extended 3 above the walking and working surface. Employee was using a portable extension-ladder for access to the roof of a single family home. Employees were exposed to a fall of approximately 13 feet while attempting to use the ladder. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
