29 CFR 1904.4(a): Each employer required by this Part to keep records of fatalities, injuries, and illnesses did not record each fatality, injury and illness that was work-related, a new case, and meets one or more of the general recording criteria:     Located at Schodack CML, LLC, on or about 10/16/2012, the employer did not record the following workplace injuries and illnesses on the OSHA 300 Log for calendar Year 2012:    (a) On or about 07/14/2012, case number OSHA-CJ-07-14, an employee fractured his foot from stepping out of his truck.  The injury involved prescription medication.    ABATEMENT CERTIFICATION REQUIRED FOR THIS ITEM
