29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Employees were exposed to laceration and amputation hazards when operating the horizontal band saw to dismantle pallets.    b) Employees operating the green stacking machine were not protected from hazards created by the rotating shaft and conveyor nip points.  The employer did not assure that a rotating shaft and in-going nip points, located at the discharge end of the chain conveyor, were guarded.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.213(a)(9): All belts, pulleys, gears, shafts, and moving parts were not guarded in accordance with the specific requirements of 1910.219:    a) The employer did not assure the belt and pulleys, which transfer power from the trim track saw motor to the saw, were guarded.  Employees were exposed to hazards created by the rotating parts and nip-points.    b) Employees were exposed to in-running nip-points and moving parts hazards of the trim track saw.  The employer did not assure that ingoing nip-points of the conveyor track, and of the chain and sprocket used to transfer power from the conveyor motor to the conveyor track, were guarded to protect employees from injury.      c) The employer did not assure that a portion of the saw blade, located behind the table, was guarded to protect employees from amputation injury.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:    a) On or about October 16, 2012, the employer did not assure that the electrical enclosure box and associated power receptacle of the green stacker was free from electrical hazards by ensuring that the equipment was in good repair and located so as not to be damaged from weather.   Employees were exposed to electrical hazards in that the disconnect box and associated power receptacle were damaged and a flexible cord was exposed to abrasion in that it was not protected from a moving drive belt.
