The belt sander (Mfr. Charles L'Hommedieu & son) did not have a guard to protect employees from the ingoing nip points created by the pulley and belt on the sander. 29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed.    Automatic Band Saw Area.  The energized electrical disconnect box (110v) on the wall behind the horizontal bandsaw had an uncovered opening on the side from a missing knockout.      There is no abatement documentation or certification required for this item.

Maintenance employee(s) who performed voltage testing on energized components up to 240v on machines such as, but not limited to, grinders and mills, were not qualified to perform testing work on electric circuits or equipment.    Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).    Abatement note:  Qualified persons shall be capable of working safely on energized circuits and shall be familiar with the proper use of special precautionary techniques, personal protective equipment (PPE), insulating & shielding materials and insulated tools (see 1910.333(c)(2)).

Electrical protective equipment was not provided or utilized, including but not limited to the following, to protect against electrical shock and arc flash/blast injuries:    a) Rubber insulating gloves with leather protectors.  b) Arc-rated long-sleeve shirt and pants or arc-rated coverall.  c) Hearing protection (canal inserts).    Abatement certification is required for instance b) only in accordance with 29 CFR 1903.19(c).  Instances a) and c) were corrected during inspection.
