29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    a) 1553 North 19th Street Bozeman, MT 59715: On October 17, 2012 and at times prior there to, employee(s) were working on a 3 tiered fabricated frame scaffold at a height of approximately 18 feet. The scaffold was not fully planked. The scaffold required 3 planks in width and only had 1 or 2 planks on each level.

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) 1553 North 19th Street Bozeman, MT 59715: On October 17, 2012 and at times prior there to, employee(s) were working on the roof of a building being constructed. The employees were installing stone on the face of the building and were not protected from a fall of approximately 12 feet to the ground below.
