29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:      On or about October 28, 2012 the employer failed to follow machine guarding requirements for machines:    a) Thermoforming machine 2: expanded metal guards near mold and cutting areas were not secured or permanently affixed to the machine.      b) Thermoforming machine 3: expanded metal guards near mold and cutting areas were not secured or permanently affixed to the machine. A piece of cardboard was taped to the side of the stacker unit and used as a guard to protect employees from moving parts. The cardboard guards were not made of durable material nor were they securely affixed.       c) Thermoforming machine 4: expanded metal guards near mold and cutting areas were not secured or permanently affixed to the machine.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).     Integrated Thermoforming Systems, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(2), which was contained in OSHA inspection number 314034661, citation number 01, item number 001 and was affirmed as a final order on July 30, 2010, with respect to a workplace located at 305 Hankes Avenue, Aurora IL 60505.
