The employer did not ensure that respirators were cleaned and disinfected using procedures in Appendix B-2 of this section, procedures recommended by the respirator manufacturer, whichever procedures are of equivalent effectiveness:      (a)	C.T. Holdings dba Northern Colorado Line-X, 1111 Diamond Valley Dr., #104, Windsor, CO 80550: On and before October 17, 2012, the employer did not provide a mechanism for employees to clean their respirators.  The employer requires respirators to be worn.  This exposes employees to isocyante inhalation due to respirators that are contaminated on the inside.     Abatement Note:  Abatement certification is required for this item (See enclosed Sample Abatement Certification Letter).

29 CFR 1910.134(h)(2)(i):  All respirators shall be stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals, and they shall be packed or stored to prevent deformation of the facepiece and exhalation valve:    (a)	C.T. Holdings dba Northern Colorado Line-X, 1111 Diamond Valley Dr., #104, Windsor, CO 80550: On and before October 17, 2012, the employer did not provide a mechanism for employees to properly store their respirators.  The employer requires respirators to be worn when applying bedliner.  This exposes employees to respiratory hazards due to poorly maintained respirators or respirators that are contaminated on the inside. A respirator was hanging on the wall outside of the spray area.     Abatement Note:  Abatement certification is not required for this item.
