29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:     (a)  On or about October 10, 2012, and at times prior, employees were exposed to fall hazards of 30 feet and greater while roofing an apartment complex without all components of their personal fall arrest systems connected and therefore not providing protection.     Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.502(d)(1): Personal fall arrest system connectors were not drop forged, pressed or formed steel, or made of equivalent materials:    (a)  On or about October 10, 2012, and at times prior, employees were exposed to fall hazards of 30 feet and greater while roofing an apartment complex with a rope attached to an anchor by the rope being tied in a knot rather than by a connector.
