29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    Jobsite located at 256 N. Madison St., Wichita, KS 67214 - Employees engaged in residential roofing activities were exposed to fall hazards of 15 feet from the roof to the ground below.  Employee was working on the 8x12 pitch roof with no fall protection methods in use.    Israel Bermudez has been previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(b)(13) which was contained in OSHA inspection number 314025057, Citation number 1, Item number 2, issued on 06/21/2010, with respect to a workplace located at 1836 S. Woodlawn Street, Wichita, KS 67226, with a final order date of 07/14/2010.    29 CFR 1903.19(d)(1) abatement of the above violation was verified at time of inspection, no certification is required.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Jobsite located at 256 N. Madison St., Wichita, KS 67214 - Employees engaged in residential roofing activities were exposed to fall hazards of 15 feet from the roof to the ground below.  Employee was working on the 8x12 pitch roof with no fall protection methods in use.  Fall protection training was not provided to each employee to recognize fall hazards and the means/methods to ensure compliance.    Israel Bermudez has been previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1) which was contained in OSHA inspection number 314025057, Citation number 1, Item number 3, issued on 06/21/2010, with respect to a workplace located at 1836 S. Woodlawn Street, Wichita, KS 67226, with a final order date of 07/14/2010.    29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
