The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: Authorized employees were not trained to acquire the knowledge and the skills to identify all energy sources to lockout/tagout machines and/or equipment, and to verify that the energy sources were isolated. a) On or about September 21, 2012, in the BOF (Basic Oxygen Furnace) area, authorized employees were tasked to perform Lockout/Tagout operations on the C2 conveyor. The #2 Secondary Damper was inadvertently deenergized instead of the conveyor, leaving the C2 conveyor not isolated. b) On or about September 25, 2012, in the BOF (Basic Oxygen Furnace) area, authorized employees were tasked to perform Lockout/Tagout operations on the C3 conveyor. The C3 conveyor was properly locked out, but the Tripper Car that straddles the conveyer had not been locked out.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) On or about September 21, 2012, in the BOF (Basic Oxygen Furnace) area, authorized employees were tasked to perform Lockout/Tagout operations on the C2 conveyor to allow contract employees to perform cleanup duties. The #2 Secondary Damper was inadvertently deenergized instead of the conveyor, leaving the C2 conveyor energized. b) On or about September 25, 2012, in the BOF (Basic Oxygen Furnace) area, authorized employees were tasked to perform Lockout/Tagout operations on the C3 conveyor to allow contract employees to perform cleanup duties. The C3 conveyor was properly locked out, but the Tripper Car that straddles the conveyer, where the contractors were working, had not been deenergized and disabled during the cleanup operations.

The Tripper Car that straddles the conveyer had not been locked out and it moved during the cleanup operations.
