The final order date was September 14, 2011.    29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    a. On or about October 17, 2012, three employees using Hitachi pneumatic air nail guns did not have safety glasses, exposing themselves to being hit by nails or flying debris.

The final order date was February 14, 2012.    29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a. On or about October 17, 2012, on a 4:12 pitch roof, three employees performing roofing operations were not protected from an 18 feet 1-inch fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.
