29 CFR 1926.304(i)(1):   Each circular hand-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut:    Jobsite: The circular hand fed rip saw that was used by employees to rip trim boards was not guarded with a hood that completely enclosed the portion of the saw above the table.

29 CFR 1926.453(b)(2)(v):   A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    Jobsite:  The employee who was utilizing an aerial lift basket was not attached to the basket or boom with a body belt or a full body harness.    Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.
