29 CFR 1910.269(c)(2)(i): An extensive briefing was not conducted in instances where the work was complicated or particularly hazardous: On or about October 15, 2012, the employer did not brief the employees performing electrical power line repairs at a job site where the system operator had not given clearance to begin repairs of the electrical lines. Employees were exposed to electrical shock hazards.

29 CFR 1910.269(m)(3)(v): After the applicable requirements in paragraphs (m)(3)(i) through (m)(3)(iv) of this section have been followed and the employee in charge of the work has been given a clearance by the system operator, the lines and equipment to be worked shall be tested to ensure that they are deenergized. On or about October 15, 2012, the person in charge of making electrical repairs of downed electrical lines had not been given clearance by the system operator to begin repairs exposing the employees to electrical shock.
