29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. On or about 10/16/2012 at 481 Northolt Parkway Suwanee, GA; employees performing framing operations were exposed to electrical hazards where grounding pins were missing from the 110 V extension cords. 1. Building 400 employees were observed, using two extension cords to power the Makita circular saws and a DeWalt Drill 2. Building 600 were observed, using an extension cord to power a circular saw on the upper level.

On or about 10/16/2012 at 481 Northolt Parkway Suwanee, GA; employees performing framing operations were exposed to potential falls of 31' 8" to the lower level where the employer failed to ensure that the employees were wearing personal fall protection. (Bld 600) 2. On or about 10/16/2012 at 481 Northolt Parkway Suwanee, GA; an employee performing framing operations was exposed to a potential fall of 8' to the lower level where the employer failed to ensure that the employees were wearing personal fall protection. (Bld 500) 3. On or about 10/26/2012 at 481 Northolt Parkway Suwanee, GA; employees performing framing operations were exposed to potential falls of 36' 7.5" to the lower level where the employer failed to ensure that the employees were wearing personal fall protection. (Bld 500) 4. On or about 10/26/2012 at 481 Northolt Parkway Suwanee, GA; an employee performing punch work operations was exposed to potential falls of 27' to the lower level where the employer failed to ensure that the employee was correctly tied off, the employee was observed with his lanyard wrapped around the framework.
