29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level:    a) North side of structure: On October 18, 2012 employees stacked concrete blocks onto a scaffold platform without a fall protection system in place.      b) North side of structure: On October 18, 2012 employees performed masonry activities on a non-stop scaffold system without an end guardrail panel in place.       c) South side of structure: On October 18, 2012 employees performed masonry activities on a non-stop scaffold system without a guardrail system in place.      Note) Abatement certification IS required for this item.

29 CFR 1926.451(h)(2)(ii):     Where there was a danger of tools, materials, or equipment falling from a scaffold and striking employees below, a toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below:    a) South end of structure: On October 18, 2012 a toe board was not in place on the working level of the non-stop scaffold system, nor was the area below barricaded to keep employees away.
