29 CFR 1926.451(c)(1)(ii): Guys, ties, and/or braces were not installed  according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet ((0.91 m) wide or less, and every 26 feet (7.9 m) or less therafter for scaffolds greater than 3 feet (0.91m) wide.    On or about October 18, 2012, at 1085 Gallatin Way, Suwanee, Georgia:    Four employees performing brick masonry work from the top level of a tubular welded frame scaffold that was approximately 20.5-feet tall, five-feet wide, and 50-feet long were exposed to fall hazard when the employer did not tie the scaffold to the buidling.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.    On or about October 18, 2012, at 1085 Gallatin Way, Suwanee, Georgia:    An employee performing brick masonry work was exposed to a struck-by hazard from trowels, bricks, and buckets of masonry cement while using a pulley system to deliver buckets of masonry cement to the employees working above him when no toe boards were installed on the work platform.

29 CFR 1910.1200(g)(1): The employer did not have a material safety data sheet for each hazardous chemical which they used: (Construction Reference: 1926.59):    On or about October 17, 2012, 1085 Gallatin Way, Suwanee, Georgia:    Employees performing brick masonry work were exposed to inhalation, and skin irritation, hazards while using but not limited to Holcim Type N Custom Color Mortamix Buff Masonry Cement which contains Portland Cement and hexavalent chromium when the employer did not have an MSDS to inform the employees how to recognize and minimize chemical hazards of the masonry cement they work with.
