29 CFR 1910.332(b)(1):     Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:      Employees who perform electrical maintenance, repairs, and lockout, did not receive training on electrical safe work practices including but not limited to:            a.  The skills and techniques necessary to distinguish exposed live parts from other parts of electric equipment.         b.  The skills and techniques necessary to determine the nominal voltage of exposed live parts, and         c.  The clearance distances specified in 1910.333(c) and the corresponding voltages to which the qualified person will be exposed.          d.  Be familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools.

29 CFR 1910.333(b)(2)(iv):     The company lockout program did not require that before circuits were worked on as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verify that the circuit elements and equipment parts were deenergized.    The lockout program did not require a qualified person to use test equipment to verify the absence of voltage when performing lockout procedures when employees were exposed to energized or potentially energized equipment.      Qualified person. One who has received training in and has demonstrated skills and knowledge in the construction and operation of electric equipment and installations and the hazards involved.

29 CFR 1910.335(a)(1)(i):     Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:      Employees performing electrical lockout procedures and electrical maintenance and repairs were not provided electrical protective equipment that was appropriate for the work being performed.  For example, the employees were not provided with electrically rated gloves to take voltage readings when verifying deenergization of circuits for electrical lockout, instead the employees had polyvinyl chloride chemical gloves.
