29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through. a) In the grain Dumping area #2 employees were exposed to trip and fall hazards in that the grating had an opening.

The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a) A written program was not established to implement the procedures, training and periodic inspection to address unexpected energizing/release of energy during serviceing, maintenance and operation of equipment including but not limited to bin sweep clean outs.
