29 CFR 1910.147(c)(4)(i):  Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.   Note: Exception: The employer need not document the required procedure for a particular machine or equipment, when all of the following elements exist: (1) The machine or equipment has no potential for stored or residual energy or reaccumulation of stored energy after shut down which could endanger employees; (2) the machine or equipment has a single energy source which can be readily identified and isolated; (3) the isolation and locking out of that energy source will completely deenergize and deactivate the machine or equipment; (4) the machine or equipment is isolated from that energy source and locked out during servicing or maintenance; (5) a single lockout device will achieve a locker-out condition; (6) the lockout device is under the exclusive control of the authorized employee performing the servicing or maintenance; (7) the servicing or maintenance does not create hazards for other employees; and (8) the employer, in utilizing this exception, has had no accidents involving the unexpected activation or reenergization of the machine or equipment during servicing or maintenance.    Agri-Fab, Inc. had an employee performing maintenance operations, die changes, on Niagra Press #31, and the employer did not develop, document, and utilize an energy control procedure for this piece of equipment.  The energy control procedure would include the following:    a.	A specific statement of the intended use of the procedure,  b.	Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy,   c.	Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them, and  d.	Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.    AGRI-FAB, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD: 29 CFR 1910.147(C)(4)(I) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 107509, CITATION NUMBER 1, ITEM NUMBER 1A AND WAS AFFIRMED AS A FINAL ORDER ON 02/14/12 WITH RESPECT TO A WORKPLACE LOCATED AT 809 S. HAMILTON, SULLIVAN, IL.

29 CFR 1910.147(c)(7)(i):  The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees.      Agri-Fab, Inc. had an employee perform maintenance operations within Niagra Press #31, and the employer did not provide lockout training to ensure the employee had the knowledge and skills to control all hazardous energy during maintenance operations.  This training would include training in the following:    a.	Recognition of applicable hazardous energy sources,   b.	Type and magnitude of energy available, and   c.	Methods and means necessary for energy isolation and control.    AGRI-FAB, INC. 29 CFR 1910.147(d)(4)(i):  Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees.    Agri-Fab, Inc. had employees exposed to amputation hazards while performing maintenance operations within mechanical power presses, and the employer did not have the employees apply an energy control device on each energy source prior to performing maintenance and servicing operations.  These instances included but are not limited to the following:      a.	Niagra Press #31 - Die changes, and   b.	Minster Press #22 - Die change on February 7, 2013.    AGRI-FAB, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD: 29 CFR 1910.147(D)(4)(I) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 107509, CITATION NUMBER 1, ITEM NUMBER 3 AND WAS AFFIRMED AS A FINAL ORDER ON 02/14/12 WITH RESPECT TO A WORKPLACE LOCATED AT 809 S.
