29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: 20 Lyman Street, West Springfield, MA, Roof; Employees were working within 3 feet of the service entrance and had run hoses from the air compressor over the electric service entrance exposing the employees to electric shock from the 220 volt electrical service entering the house.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support). 20 Lyman Street, West Springfield, MA, roof; Employees were accessing the roof of the single story residence from an aluminum extension ladder that was not set at the 4:1 rise to run angle and required by the manufacturer per the pictogram on the ladder and the OSHA standards.
