Powered Indsustrial Vehicle Area - On or about October 18, 2012, in the powered industrial vehicle charging areas, employees were required to charge the batteries on the powered industrial vehicles. The damaged part(s) which included the insulation on the charging cables for the batteries exposed employees to electrical hazards.
