29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely: a/ E.S. Sutton/Sayreville, NJ Site - Warehouse Aisle E Storage Area: "Crown" #2 and #29 Electric Hand Jacks (Known as "Walkie-Riders") collided while being operated. The training provided by the employer was inadequate in that 1/ the training provided did not prevent operator(s) from using electronic devices that would prevent them from observing lift truck traffic. 2/ the training did not prevent operator(s) from carrying loads that prevented their visibility during operation.at an intersection in Aisle E of the warehouse. Observed on/or about 10/18/2012.
