29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: JOB SITE: 10734 Tesshire Drive, Affton, MO 63123, at this work site,the employer failed to provide an effective training program to instruct his workers in identifying hazards, avoiding unsafe work conditions and control or eliminate any hazards and/or exposure to illness. The employer allowed his workers to used an extension cord with the strain relief torn away from the female connection, install new roof shingles without adequate form of fall protection, and use an aluminum ladder with the ladder side rails less than 3 feet above the upper landing surface.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: JOB SITE: 10734 Tesshire Drive, Affton, MO 63123, at this work site, an aluminum ladder was used to gain access on and off the roof and the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed. The workers were exposed to fall hazard.
