29 CFR 1910.23(a)(8):     Every floor hole into which persons can accidentally walk was not guarded:    Location:  At or near 899 Veterans Blvd, Festus, MO  63028.    The employer did not ensure that the floor drain next to the cooler in the back of the store was covered to prevent workers from stepping into the opening.  The opening measured approximately 7.5" by 4.5".
