29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:      JOB SITE: 12482 Glengate Drive, Maryland Heights, MO 63043, at this work site, a worker was observed on top of a 6:12 pitch roof, actively installing black felt paper around an open hole which was not protected from falling through the hole by personal fall arrest systems, covers, or guardrail systems erected around such hole.  The worker was exposed to a fall hazard.

The workers were exposed to a fall hazard.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:      JOB SITE: 12482 Glengate Drive, Maryland Heights, MO 63043, at this work site, a Louisville aluminum extension ladder, Model: D2222-24, was taken apart and used on two separate areas to gain access onto the roof and ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  The workers were exposed to a fall hazard.

29 CFR 1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components:      JOB SITE: 12482 Glengate Drive, Maryland Heights, MO 63043, at this work site, a Louisville aluminum extension ladder, Model: D2222-24, was used to gain access onto the roof, approximately 13 feet 3 inches, was structurally defective with the left foot of the ladder bent approximately 45 degrees inward and with a bend rung.  The workers were exposed to a fall hazard.
