29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about 10/18/12, on the east side of building #4, an employee was leaning over the edge of a flat roof repairing an external light at an approximate height of 18 feet to the ground below exposing the employee to fall hazards.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about 10/18/12, on the east side of building #4, an employee was not trained in the recognition of fall hazards in that the employee was leaning over the edge of a flat roof repairing an external light at an approximate height of 18 feet to the ground below and was unaware of the need or requirement to have fall protection exposing the employee to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about 10/18/12, on the east side of building #4, an employee accessed a flat roof with a 24 foot extension ladder in order to repair an exterior light at an approximate height of 18 feet to the ground below and the ladder did not extend 3 feet past the landing to ensure a safe means of access exposing the employee to fall hazards.
