29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    This violation occurred on October 18, 2012, where at least one (1) employee, was exposed to struck-by hazards from falling material or equipment and was not protected by the use of a protective helmet.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.    This violation occurred on October 18, 2012, at the jobsite located at 4949 Westgrove Drive in Dallas, Texas 75248, where at least three (3) employees that were performing roofing work did not use lanyards in conjunction with safety harnesses.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:     This violation occurred on October 18, 2012, at a jobsite located at 4949 Westgrove Drive in Dallas, Texas 75248, where the side-rails of the ladder that at least three (3) employees used to access/egress the roof did not extend at least 3-feet above the upper landing surface.
