29 CFR 1910.147(c)(1):     Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.      a) The employer did not develop procedures and implement training with respect to the use of lockout in preventing release of unexpected energy during the servicing and maintenance of equipment including but not limited to the oiling and greasing of the wire and looper machine.

All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).      a) On or about October 5, 2012 electrical disconnect was not shut off to disable power on the wire machines during oiling and greasing procedures.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.      a) Machine guarding was not provided on the Amrox bandsaw to prevent employees exposure to the unused portion of the saw blade.    b) Machine guarding was not provided on Wire machine #1 to prevent employees exposure to the claw/graber arm, wire looping, suction table arm, gears and connecting rods movement.    c) Machine guarding was not provided on Wire machine #2 to prevent employees exposure to the claw/graber arm, wire looping, suction table arm, gears and connecting rods movement.    d) Machine guarding was not provided on Wire machine #3 to prevent employees exposure to the claw/graber, wire looping,   suction table arm, gears and connecting rods movement.    e) Machine guarding was not provided on the Looper machine to prevent employees exposure to suction table arm, gears and connecting rods movement.

29 CFR 1910.219(c)(2)(i):     All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires.    a) Shafts attached to the motor drive on Wire machine #1 was not guarded expoing employees to rotating parts.  b) Shafts attached to the motor drive on Wire machine #2 was not guarded expoing employees to rotating parts.  c) Shafts attached to the motor drive on Wire machine #3 was not guarded exposing employees to rotating parts.  e) Shaft attached to the motor drive on the Thread rewinder machine was not guarded exposing employees to rotating parts.

29 CFR 1910.219(e)(3)(i):     Vertical and inclined belts shall be enclosed by a guard conforming to standards in paragraphs (m) and (o) of this section.    a) Main motor drive belts on the 1/4 inch eyelet/riveter machine were not guarded exposing employees to pinch points.  b) Main motor drive belts on the 3/8 inch eyelet/riveter machine were not guarded exposing employees to pinch points.  c) Main motor drive belts on the 5/16 inch eyelet/riveter machine were not guarded exposing employees to pinch points.
