29 CFR 1926.451(g)(1)(vii):   For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  a) Jobsite -1709 Mifflin Street - Employees working on the tubular welded frame scaffold performing preparation to apply stucco and were exposed to fall hazards of approximately 13 feet without the use of a guardrail system or personal fall arrest system.  Employees were working on the 2nd and 4th levels of the scaffold on or about 10/17/12.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(g)(4)(i):   Guardrail systems were not installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.  a) Jobsite - 1709 Mifflin Street - Employees working on the tubular welded frame scaffold preparing to install stucco to the building and were exposed to fall hazards of approximately 13 feet  without the use of a guardrail system or personal fall arrest system.  Employees were working on the 2nd level and the outrigger section of the 3rd levels of the scaffold on or about 10/17/12.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    a) Jobsite -1709 Mifflin Street-A ladder or other safe means of access was not provided to employees working on the tubular welded frame scaffold.  Employees were using the side frames and cross bracing to access the working levels. Observed on or about 10/17/12.       Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(b)(1):       Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as follows:    a) Jobsite - 1709 Mifflin Street - The working levels of the tubular welded frame scaffold were not fully planked between the front and rear uprights.  Employees were working on the 2nd & 4th levels of the scaffold.  Observed on about 10/17/12.       Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement and states employees and their representatives were informed of this abatement.
