29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On October 16, 2012 at a jobsite located at 402 Richards Street Geneva IL, employees were performing roofing work and were not utilizing any means of fall protection. Employees were exposed to a fall hazard of approximately 21.4 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: On October 16, 2012 at a jobsite located at 402 Richards Street Geneva IL, employees were using the top and bottom parts of an extension ladder as straight ladders to access the roof in the back of the home. Employees were exposed to fall hazards associated with using ladders other than their intended use. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
