29 CFR 1926.21(b)(6)(i):  All employees required to enter into confined or enclosed spaces were not instructed as to the nature of the hazards involved, in the necessary precautions to be taken, and in the use of protective and emergency equipment required. The employer shall comply with any specific regulations that apply to work in dangerous or potentially dangerous areas.    On or about 20 Sept 2012 and times prior to, employees entered confined spaces in elevator pits #2, #3, #4, and #7 without being provided information of the associated hazards, equipment or necessary precaution procedures when working in areas having the following:    1) mechanical hazards,   2) electrical hazards,   3) overhead hazards,

29 CFR 1926.451(h)(2)(ii):  A toeboard shall be erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below, except on float (ship) scaffolds where an edging of 3/4 x 1 1/2 inch (2 x 4 cm) wood or equivalent may be used in lieu of toeboards;    On or about September 20, 2012 and in prior thereto, the employer did not ensure that the platform edge on the false car located more than ten feet above lower level did not have toeboards.

29 CFR 1926.454(b): The employer did not have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold be trained by a competent person to recognize any hazards associated with the work in question and as outlined in 29 CFR 1926.454(b)(1)-(4):    On or about 20 Sept 2012 and time there to the employer did not ensure that employees were trained by a competent person in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the correct procedures to control or minimize the hazards exposing employees to fall and overhead hazards during installation and use of an elevator false car.
