29 CFR 1910.27(d)(2): Fixed ladder(s) used to ascend to heights exceeding 20 feet were not provided with an offset landing platform for each 20 feet of height or fraction thereof that required the climber to exit the ladder to continue the climb: Fixed ladders exceeding 20 feet did not have an offset landing platform every 20 feet to protect employees who needed to climb them to check grain levels in the bins and the perform maintenance on the outside bucket legs exposing them to a fall of approximately 125 feet and 145 feet.
