Worksite:  The employer did not ensure through providing successful training and evaluation that all employees, who were allowed to operate the rough terrain forklift (PIT) were competent enough to operate the PIT safely and appropriately at all times.

Worksite:  The employees, engaged in roofing activities on the 4 in 12 pitched roof were not protected from the hazard of falling approximately 25 feet to the ground by use of guardrail systems, personal fall arrest systems and/or other safe means.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails could not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access, the ladder was not secured at its top to a rigid support and a grasping device was not provided.    Worksite:  The employer did not ensure that the extension ladders, which were used by employees to access the roofs were properly secured and appropriate grasping devices were provided at top.
