Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a.) VA Hospital Richmond: On or about 10/16/12, an employee was exposed to a 38' 6" fall while working on an open sided roof and fall protection was not provided.

Listed, labeled, or certified equipment shall be installed and used in accordance with instructions included in the listing, labeling, or certification.    a.) VA Hospital Richmond Poly Neuroscience Center: On or about 10/16/12, extension cords and GFCI pigtails were not used in accordance with its listing and labeling in that two extensions and two three way GFCI pigtails were daisy chained together.
