On or about October 18, 2012, on the 6:12 pitch roof, an employee using a pneumatic nailer to install shingles was not wearing safety glasses, exposing himself to eye injuries.

On or about October 18, 2012, the 28-foot extension ladder used to access the roof had a bent footing, exposing employees to a 15 feet fall hazard.

Align Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 315482133, citation 01, item number 001, issued on December 12, 2011, with respect to a workplace at 923-1st Avenue South, Jacksonville Beach, Florida 32250. The final order date was January 28, 2012.    29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     a.  On or about October 18, 2012, on the 6:12 pitch roof, employees installing shingles were not protected from a 24 feet fall hazard by the use of a fall protection system.
