29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level.        Location: Jobsite, 335 Phillips Park Road      a) Employees doing exterior stucco work were working from a tubular welded frame scaffold without fall protection, thus exposing employees to falling up to 24 feet to the ground below on or about 10/18/12.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.      Location: Jobsite, 335 Phillips Park Road    a) Employees doing exterior masonry work from a Tubular Welded Frame Scaffold were climbing down the scaffold using the cross bracing. This condition exposed employees to the hazard of falling up to 24 feet to the ground below on or about 10/18/12.

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.        Location: Jobsite, 335 Phillips Park Road    a) Employees doing exterior masonry work from a Tubular Welded Frame Scaffold were working without fall protection and egressing from the scaffold by climbing down the cross bracing. Employees had not been trained to recognize and avoid hazards while working on the scaffold, thus exposing employees to falling up to 24 feet to the ground below on or about 10/18/12.
