29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns. On or about October 22, 2012, on the east side of the construction site, an employee engaged in handing trusses up to the second level of a commercial building to another employee worked without head protection exposing the employee to struck-by hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On or about October 22, 2012, the employees, installing trusses on a commercial construction site, utilized an extension cord without a strain relief device, to energize a Porter Cable skill saw. The employees were exposed to electrical hazards.
