29 CFR 1926.652(g)(1)(iii): Employees shall be protected from the hazard of cave-ins when entering or exiting the areas protected by shields. a) Adjacent to 4723 Rt. 309 North, Worksite - on or about October 18, 2012, an employee, working in a trench shield in excess of 11 feet below grade, had to exit the trench shield's protected area to access an extension ladder installed for egressing the trench.

29 CFR 1926.652(a): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): a) Adjacent to 4723 Rt. 309 North, Worksite - on or about October 18, 2012, employees are working in a trench, in excess of 11 feet below grade, which include an inadequately installed protective system, which will not fully prevent potentially fatal crushing injuries in the event of a sudden collapse. R.G. CICCONE DEVELOPMENT, LP, WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.652(a), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312979164, CITATION NUMBER 1, ITEM NUMBER 4, AND WAS AFFIRMED AS A FINAL ORDER ON APRIL 26, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT DELONG AVENUE, BETHLEHEM, PA 18018.
