29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) Approximately 1 mile west of Georgiana, AL, on the north side of Hwy 106: On October 22, 2012; the employer failed to ensure that an employee working from an extensible boom basket was wearing fall protection, exposing an employee to falls of approximately 15 feet while drilling holes in the pole to attach a bolt.

29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedures to control or minimize those hazards. (a) One mile west of Georgiana, AL, on the north side of Hwy 106: On October 22, 2012 and at times prior to; the employer failed to provide fall protection training to all employees who might be exposed to fall hazards, exposing employees to falls of at least 15 feet while being elevated in baskets of truck lifts.
