29 CFR 1926.404(a)(2): A grounded conductor was attached to a terminal or lead so as to reverse designated polarity: a) On or about 10/16/12, at the Moog Inc. Corporate Headquarters Construction Site, Elma, NY: Employees used fluorescent light fixtures to provide lighting in an elevator shaft. These light fixtures were connected to 120 volt-ac power via a series of extension cords, the first of which was plugged into a receptacle in a 4-receptacle outlet box mounted on a wooden 2x4 in the lower level main office area inside the building. That first cord in the series was a short black GFCI 3-Way belonging to another contractor. All 3 receptacles on this cord had "reversed polarity". NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) On or about 10/16/12, at the Moog Inc. Corporate Headquarters Construction Site, Elma, NY: Employees used fluorescent light fixtures to provide lighting in an elevator shaft. These metallic light fixtures were connected to 120 volt-ac power via a series of extension cords, the first of which was plugged into a receptacle in a 4-receptacle outlet box mounted on a wooden 2x4 in the lower level main office area inside the building. That first cord in the series was a short black GFCI 3-Way belonging to another contractor. All 3 receptacles on this cord were "ungrounded" because the grounding prong was missing on its' plug. NO ABATEMENT CERTIFICATION REQUIRED
