29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition. Where wet processes were used, drainage was not maintained, and false floors, platforms, mats, or other dry standing places were not provided when practicable:    a) On or about October 23, 2012, the employer did not assure that the restaurants kitchen floor areas were maintained in so far as possible a dry condition.  Floor mats that were wet due to an overflowing drain, and areas not covered by mats, exposed employees to slip/fall hazards.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.145(c)(2)(i): Caution signs were not used to warn against potential hazards or to caution against unsafe practices:    a) The employer did not assure that caution signs were used to warn employees of potential slip, trip, and fall hazards in kitchen areas.  Signs that cautioned employees of the sloping floor when entering the kitchen near the pick-up line and also of wet floors near the pots/pans dishwashing area were not used during food service hours.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and/or 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness occurred:    a) On or about October 23, 2012, the employer did not meet requirements for recording injuries and illnesses in a timely manner.  Each recordable injury was not entered on the OSHA 300 Log (or equivalent) within seven calendar days of notification of the recordable injury.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
