29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: (a) Facility, Filler/Rinse Area, New Kensington, PA: On or about 10/23/12 the flexible cord for the coder machine did not have strain relief exposing the conductors. This condition exposed employees working in the area to potential electrical shock hazards. (b) Facility, Bottling Area, New Kensington, PA: On or about 10/23/12 the flexible cord on the Baldor motor powering the speed controls for the conveyor did not have strain relief exposing the conductors. This condition exposed employees working in the area to potential electrical shock hazards. (c) Facility, Labeler Area, New Kensington, PA: On or about 10/23/12 the flexible cord on the motor controls for the Sonic Air System blower did not have strain relief exposing the conductors. This condition exposed employees working in the area to potential electrical shock hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) Facility, Bottling Area, New Kensington, PA: On or about 10/23/12 a re-locatable power strip was not used in accordance with its UL listing and labeling, exposing employees to shock hazards. The device was being used in a wet location and in a manufacturing setting to power a sump pump. The employer, Castle Co-Packers, was previously cited for a violation of this Occupational Safety and Health Administration standard or its equivalent standard, 1910.303(b)(2) which was contained in OSHA inspection number 312632268, Citation Number 1, Item 3a, issued on 04/07/2009 with respect to a workplace located at 204B Schreiber Industrial Park, New Kensington, PA.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): (a) Facility, 204B Schreiber Industrial Park, New Kensington, PA: On or about 10/23/12 visual inspections of portable cord and plug connected equipment and flexible cords were not conducted before use for external defects. The following equipment had external defects and had not been inspected and removed from service: Cord on the portable coder machine, flexible cord on the Baldor motor, flexible cord on the Sonic Air System blower.
