29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no more than one inch:    a) North Scaffold -  A stucco mason was exposed to a 14 foot fall from a supported scaffold whose work platform was not tightly planked between the scaffolds uprights, on or about 10/12/2012.    b) East Scaffolds - A stucco masons were exposed to a 14 foot fall from a supported scaffold whose work platform was not tightly planked between the scaffolds uprights, on or about 10/12/2012.    c)  South Scaffold - A stucco masons were exposed to a 13 foot fall from a supported scaffold whose work platform was not tightly planked between the scaffolds uprights, on or about 10/12/2012.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:    a) North Scaffold -  A stucco mason was exposed to a 14 foot fall from a supported scaffold whose work platform had not been provided with proper ladder access, on or about 10/12/2012.    b) East Scaffolds - A stucco masons were exposed to a 14 foot fall from a supported scaffold whose work platform had not been provided with proper ladder access, on or about 10/12/2012.    c)  South Scaffold - A stucco masons were exposed to a 13 foot fall from a supported scaffold whose work platform had not been provided with proper ladder access, on or about 10/12/2012.

29 CFR 1926.404(b)(1)(i):     Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:      a)  Rear of House, Ground Level - Masons using a 7  inch Milwaukee portable circular saw to cut lumber, were exposed to a 120 Volt electrical shock hazard for the saw they were using in the course of their duties was not provided with an assured equipment grounding conductor program of ground fault circuit interrupter circuitry, on or about 10/12/2012.
