Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Jobsite: 6400 Loretto Ave. Philadelphia, PA- An employee was exposed to a fall hazard of approximately 15 feet while removing shingles from a steep roof without any fall protection. Observed on or about 10/23/12. NOTE: No abatement certification or documentation required.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a) Jobsite: 6400 Loretto Ave.Philadelphia, PA - Employeer did not provide each employee fall protection training. One employee was exposed to a fall hazard without any fall protection, while removing shingles from a steep roof. On or about 10/23/12. NOTE: Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
