29 CFR 1910.305(g)(1)(iv)(C): Flexible cords and cables cannot be used when run through doorways, windows, or similar openings: On or about October 23, 2012, a flexible cord was in use improperly as it was observed running through two doorways to power a space heater in the mechanical room on the 15th floor. Both doors were observed nearly closed, pinning the cord between the door and the door jamb.

29 CFR 1910.1001(j)(4)(i): The building owner, facility owner or employer identified previously installed presumed asbestos containing materials (ACM) and/or presumed asbestos containing materials (PACM) and did not affix or post labels or signs so that employees would be notified of what material contains ACM and/or PACM: On or about October 23, 2012, in several areas of the building such as, but not limited to, the installed ACM on the pipe wrap in the basement boiler room and ACM in the 15th floor mechanical room, no labels or other means of notification were apparent indicating the presence of the asbestos in order to inform workers that have access to and work in these areas of the appropriate hazards.
