29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) Northside, Upper roof - Employees cleaning and removing debris 9 feet from a lower level without fall protection; on or about 10/22/12.    Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)    a) Worksite - Employees cleaning and removing debris from the upper roof approximately 9 feet from the roof below without fall protection. The employees were not trained on reconizing fall hazards; on or about 10/22/12.    Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) Northwest corner - Employees were using an unsecured ladder to gain access to the first roof. The ladder was approximately 1 inch above the upper landing; 10/22/12.    b) North side, Lower Roof- Employees were using an unsecured ladder to gain access to the second roof. The ladder was approximately 3 feet below the upper landing; on or about 10/22/12.    Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

The employee's back was against the ladder; on or about 10/22/12.
